In order to streamline check-in and give the most accurate payment options at your first visit, we request that you complete the appropriate New Patient Form (Child or Adult) below at least 2-3 days prior to the appointment.

During your first visit, we will introduce you to our practice and staff. We will take photo and x-ray records. The doctor will complete an exam and discuss treatment options. We will present payment options and discuss customizing a payment plan for your needs.

We accept all major insurances. We also offer flexible payment plans and low down payments. Please keep in mind that all treatment plans are custom designed for each individual and treatment costs vary depending on the type of treatment necessary.
